What treatments for frequent heartburn are actually supported by clinical research? Many options online seem to rely mostly on personal testimonials. Is Omeprazole safe for long-term use? I’m 70—does age affect the safety or choice of treatment?

Low stomach acid is commonly observed in individuals aged 50 and above. The use of medications such as omeprazole, which reduce stomach acid, can exacerbate this condition. As a result, food would not be properly digested. Undigested food can undergo fermentation, producing toxins that can contribute to heartburn. In chronic cases, there can be overgrowth of H. pylori and other bacteria in the stomach and intestines.
Low stomach acid can also occur in individuals younger than 50. In clinical settings, heartburn is rarely caused by excessive stomach acid, it is caused by low stomach acid. Therefore, omeprazole is not necessary.
I suggest to assess stomach acid levels through a Gastrotest. If low stomach acid is confirmed, betaine HCl is often recommended. However, it is important to first ensure that the stomach and intestinal lining are intact. In the presence of ulcers or intestinal inflammation, betaine HCl is not appropriate. A more practical approach to supporting digestion is the intake of lemon juice 20–30 minutes before meals. This can aid digestion, as the acid from lemon is utilized by the body and have a milder effect on the gut lining compared to betaine HCl.
Additionally, a stool test can help determine the presence of H. pylori or other harmful microorganisms. Individuals who have taken omeprazole are more likely to have such microbial overgrowth and testing can help confirm this. In general, microbes can be addressed using broad-spectrum botanical antimicrobials such as oregano and clove, which act against various types of microorganisms. Black walnut is commonly used for parasites, while cinnamon is used for fungal organisms. Some people do not response to these natural remedies so antimicrobial drug can be used for short period of time about 2 weeks then always followed by probiotics.
Doctors usually recommend oregano supplements with olive oil in the ingredients. However, some individuals may not tolerate these well, particularly if liver function is compromised as I have personally experienced. Once liver is fully healed, one can tolerate olive oil. But it also depends for some individuals. I recall that my brother previously complained of swollen gums and fever. I advised him to take an oregano supplement along with gargling coconut oil combined with antimicrobial essential oils, including oregano. His symptoms resolved within a few days, and he did not report any adverse effects, despite being alcoholic.
